Take I-264 to the Taylor Blvd. Exit. Turn south on Taylor Blvd. and follow Taylor to Bluegrass Avenue. Turn right on Bluegrass Avenue and follow Bluegrass Avenue to Churchman Avenue (just before Sts. Mary and Elizabeth Hospital). Turn right on Churchman and take an immediate left turn into the Hazelwood Center entrance drive. Follow the drive to the top of the hill, bearing right as you go. At the top of the hill, you will Hazelwood Center's main building. Enter at the main entrance and you will be directed to the Underwood and Lee Clinic.
